Jack Lee Smith, 57, of Nelsonville died April 2, 2006, at White River Medical Cener in Batesville.
He was born Aug. 8, 1948, at Miami, Okla., the son of Donald Leroy and Priscilla Ellen Garrett Smith. He was employed by the Bank of Cave City, served in the United States Army during the Vietnam War and was of the Church of Christ faith.
Survivors include his wife, Patricia Smith of the home; one daughter, Stacy Smith of Bremerton, Wash.; his mother and stepfather, Pat and Wayne Stout of Batesville; one brother, Ed Smith of Nelsonville; two sisters, May Turner of Davis, Calif. and Priscilla Bobst of Modesto, Calif.; one stepbrother, Steven Stout of Newark; two stepsisters, Linda Phillip of Russellville, and Teresa Barnett of Batesville; one granddaughter, Alena Smith of Bremerton, Wash.; several nieces and nephews; and a host of other relatives and friends.
He was prceded in death by his father, Donald LeRoy Smith; and one sister, Ester Colleen Smith.
A private memorial service will be held at a later date, with arrangements under the direction of Qualls Funeral Home of Cave City.
