Health Department inspections

The following are the results of inspections conducted through March 27 by the Arkansas Health Department. The Ash Flat office conducts inspections in Sharp, Fulton and Izard counties.

Sonic of Mammoth: Hwy. 63 and Hill Street, Mammoth Spring. Date of inspection March 19, 10:10 a.m. Corrective action needed: Clean above ice cream machine. Clean shelves above three comp. sink. Clean vent hoods above fryers and grill. Clean bins that hold fry boxes, etc.

Woods Riverbend Restaurant: 80 Main, Mammoth Spring. Date of inspection March 19, 11 a.m. Corrective action needed: Keep door on ice machine closed. Need handwashing sign in women's restroom - COS. Temp of walk-in cooler should maintain food at 41 degrees F or below. You have until April 2010 to correct. Store ice scoop on sanitized surface. Employee drinks need lids and straws. Clean inside and outside of microwaves. Sausage should be held at 41 degrees F or below or 135 degrees F or above. Observed at 73 degrees F. Clean floor and shelves in food prep area as well as ceiling and light fixtures.

The Dawg House: 438 Hwy. 63, Mammoth Spring. Date of inspection March 19, 11 a.m. Corrective action needed: Items prepared and stored more than 24 hours should be labeled with name and prep date. Wiping cloths are required to be in 50 ppm bleach or equivalent sanitizer when not in use. Store ice off floor if walk-in freezer.

Hog Wild Pizza: 201 Main, Mammoth Spring. Follow-up inspection. Date of inspection March 19, 1:25 p.m. Critical items have been corrected or addressed at time of inspection. Install utility sink by April 16, contact plumber. Note: provided handwash signs to aid in maintaining hand sink for hands only.

Cozumel Restaurant: 4200 Hwy. 62/412. Hardy. Follow-up inspection. Date of inspection March 17, 1 p.m. Corrective action needed: Dishwasher is not reaching proper temp for sanitizing dishes. Repair/replace dishwasher. Until then, sanitize after washing with 50 ppm bleach or equivalent. All other violations observed in previous inspections have been corrected at this time.

McDonald's: 400 Hwy. 63, Hardy. Follow-up inspection. Date of inspection March 17, 11 a.m. Violations observed at time of previous inspection have been corrected at time of inspection.

Dollar General 9255: 1797 Hwy. 62, Hardy. Date of inspection March 16, 12:15 p.m. No violations observed at time of inspection.

KFC 12: Hwy. 62/412. Hardy. Date of inspection March 16, 12:40 p.m. Corrective action needed: Repair damaged tiles in food prep and storage area. Clean floor of food prep area.

China Garden: 2002 Hwy. 62, Hardy. Follow-up inspection. Date of inspection March 16. Corrective action needed: Do not store personal items (coats, hats, etc.) on shelves with food - Repeat. Limit number of eggs on counter. One flat is plenty, refrigerate others - Repeat. Employee drinks need lids and straws - Repeat. Do not store knives in cracks between equipment. Raw meat stored above ready-to-eat foods in walk-in cooler - COS.

Senior Citizens Center: Town Center, Cherokee Village. Corrective action needed: Ice machine needs to be cleaned. Clean top of refrigerator. Meals prepared at Izard County Senior Center.

Lakeview Dinette: 200 Iroqouis, Cherokee Village. Date of inspection March 13, 10:30 a.m. Corrective action needed: Wiping cloths should be stored in sanitizer equal to 50 ppm bleach when not in use. Lower temp on prep cooler to 41 degrees F or below -- observed at 45 degrees F. Need self closure on employee restroom. Dispose of rotten apples in walk-in cooler - COS.

Clyde's Chuck Wagon: Hwy. 62/412, Cherokee Village. Complaint. Date of inspection March 13, 11 a.m. Corrective action needed: Children are being kept in food prep area - Repeat - Complaint filed 3/10. Employee drinks need lids and straws - Repeat. Do not store pitchers on towels. Label all spray bottles. Repair damaged chop block -- cracks and holes are impossible to properly sanitize. Shielded lights required in walk-in freezer. Store food off floor of walk-in freezer - Repeat. Ice Maker should drain to sewer - promotes mosquitoes and West nile. Boxes should be disposed of in dumpster. Raw pork butt shoulder stored on same shelf with fresh cabbage, potatoes and onions. Raw foods are to be stored below ready-to-eat foods. Prepared ribs require date marking and keep covered. Employee returned from child holding area did not wash hands and returned to work.

Note: The owner/PIC of this facility should come to the Sharp County Health Unit on March 18 for the development of a corrective action plan. This plan will address chronic violations of the food code and provide instructions for compliance as well as education as to why these rules are important in preventing food borne illnesses.

Evening Shade School: 200 School Street, Evening Shade. Date of inspection March 20, 10:10 a.m. Corrective action needed: Replace missing end cap on light shield in store room. Plastic ware should be stored handle up.

Cave City Head Start: 217 School Road, Cave City. Date of inspection March 20, 10:15 a.m. Corrective action needed: Some spray bottles need relabeling. Meals prepared at Cave City Elementary.

Cave City School: Main Street, Cave City. Date of inspection March 20, 11 a.m. No violations observed at time of inspection.

Cave City High School: 1711 N. Main Street, Cave City. Date of inspection March 20, 11:25 a.m. No violations observed at time of inspection.

First Assembly Day Care: 437 N. Main, Cave City. Date of inspection March 20, 11:38 a.m. Corrective action needed: Clean inside of freezer.

Rebel Time Out II: 2375 Hwy. 62/412, Hardy. Date of inspection March 23, 1 p.m. Correction action needed: Hand sink should be used for hand washing only. Vent hood required above grill and fryers -- contact HVAC for specific requirements. Store soda, crackers, etc., below laundry detergent - COS. Provide covered waste can in women's restroom. Wiping cloths should be stored in sanitizer equal to 50 ppm bleach.

Rebel Station: 4201 Hwy. 62, Hardy. Date of inspection March 23, 1:35 p.m. Corrective action needed: Repair self-closing door on women's restroom.

Lela's Cakes: 352 Morgan, Hardy. Date of inspection March 23, 2 p.m. No violations observed at time of inspection. Facility was not in use at time of inspection.

Price Chopper 367: 202 W. Main, Hardy. Date of inspection March 24, 10:34 a.m. Corrective action needed: Severly dented cans should not be sold even at a discount. Evidence of rodents on shelves - Repeat. Lower temp of bologna bin and vegetable display to maintain foods at 41 degrees F or below. Replace missing light shields in meat walk-in cooler. Discard all foods in veg cooler that has been temperature abused. Clean vent fans in meat cooler. Wiping cloths in meat department should be stored in sanitizer equal to 50 ppm bleach when not in use. Replace missing end caps on lights in meat department.

Price Chopper Deli/Pizza: 202 W. Main, Hardy. Date of inspection March 24, 11:08 a.m. Corrective action needed: Hand wash sink should be used for handwashing only - COS. Deli area has been moved - plans were not sent in - facility required to have a three compartment sink to wash, rinse and sanitize utensils. Handsink is required to have hot water.

Hardy Senior Center: 2nd and Johnson, Hardy. Date of inspection March 24, 11:49 a.m. No violation observed at time of inspection. Meals prepared by Izard County Senior Center.

Negron's Bakery: 2110 Hwy. 62/412. Hardy. Date of inspection March 24, 9:10 a.m. Corrective action needed: Wiping cloths should be stored in sanitizer equal to 50 ppm bleach when not in use. Cooked steak is still potentially hazardous and should be stored below 41 degrees F or above 135 degrees F - if cooling, foods should be cooled rapidly to 71 degrees F in two hours and 41 degrees F in another two hours. Employee drinks are required to have lids and straws. Coffee creamer should be refrigerated below 41 degrees F or use creamer that is shelf stable.

Oxford One Stop: Hwy. 9 and 354, Oxford. Date of inspection March 25. Corrective action needed: Tomatoes should be stored at 41 degrees F or below. Discard broken glass bottle of hot sauce. Clean top of refrigerator. Wiping cloths should be stored in sanitizer equal to 50 ppm bleach. Sausage in freezer from custom butcher should not be used or sold at retail -- take home. Discard boxes of napkins that were damaged by water leak.

Liz's Chatterbox: Hwy. 9 and 354, Oxford. Date of inspection March 25. Corrective action needed: Ice should be stored in food grade bags -- currently garbage bags. Clean inside of microwave. Store Half & Half coffee creamer at 41 degrees F or below or use shelf stable creamer.

Carriage Room: 900 4th Street, Horseshoe Bend. Date of inspection March 25. Corrective action needed: Foods prepared and stored longer than 24 hours should be labeled with name and date of prep. Can opener blade should be cleaned and sanitized daily. Raw shrip/prawns stored above raw mushrooms in walk-in cooler. Keep foods covered in walk-in cooler. Store foods off floor of walk-in freezer a min. of 6 inches -- repeat. Garbage bags should not be used as liners for bulk dry goods -- use food grade plastic liners. Floor of dry storage needs swept, leaves have blown inside. Handsink should be kept clean at all times. Facility was not open for business at time of inspection. Should have consumer advisory posted on menu or tables indicating consumption of raw foods (ie: oysters on half shell) may increase risk of food borne illness especially in people with certain medical conditions. Refer to variance or food code for more information.

Snappy Mart 15: 400 E. Main, Horseshoe Bend: Date of inspection March 25. Follow-up inspection. Critical violations observed in previous inspections have been corrected or addressed at time of inspection.

Calabama: Hwy. 56 and 289, Franklin. Date of inspection March 25. Follow-up inspection. Violations observed in previous inspection have been corrected or are being addressed at time of inspection.

Melbourne High School: 2270 Lacrosse Road, Melbourne. Date of inspection March 25. Follow-up inspection. Corrective action needed: Dishwasher has not been repaired. Temp of rinse water must reach 180 degrees F in order to effectively sanitize. Separately sanitize until repaired in solution equal to 50 ppm bleach. Other critical violations observed have been corrected at time of inspection.

Salem Dairy Queen: Hwy. 63, Salem. Date of inspection March 26. Corrective action needed: Dipper well should be adjusted to maintain a constant outflow of water. Tomato (sliced) should be held at 41 degrees F or below -- repeat.

Sonic of Salem: Hwy. 62 and 9, Salem. Date of inspection March 26, 1:40 p.m. Corrective action needed: Wiping cloths should be stored in sanitizer equal to 50 ppm bleach. Employee drinks are required to have lids and straws. Do not store dry good bins under drain board of 3 comp sink. Blade of can opener needs cleaning. Repair thermometer in walk-in cooler and walk-in freezer. Install light shield in walk-in cooler. Lower temp of walk-in freezer to 0-10 degrees F. Dumpsters are required to have lids and remain closed. Food should not be stored in an unused restroom. Ice scoop should be stored on a surface that is cleaned and sanitized daily. Ice milk for ice cream should be stored at 41 degrees F or below. Repair leak in waitress handsinnk. Scoop should be used to retrieve ice from machine -- do not use bucket. Temp of ice cream machine incorrectly recorded.

Fun Days: Hwy. 2235, Viola. Date of inspection March 26. Corrective action needed: Utensils should be dried handle up. Three compartment sink should be used to wash, rinse and sanitize. Wiping cloths should be stored in sanitizer solution equal to 50-100 ppm bleach. Utensils should not be stored on towels. Handsink should be cleaned, supplied with soap, paper towels and used for hand washing only. Raw hamburger stored uncovered and in contact with head of lettuce and above another head of lettuce. Label all dry goods. Clean inside of microwaves. Inside of cabinets should be cleaned extensively. Cutting board needs cleaned and resurfaced. Discard shattered fluorescent light bulb. Provide scoops for dry goods with handles. Tomato, ham, etc., in prep cooler should be stored at 41 degrees F or below at all times -- use ice baths to maintain temp until prep cooler can be replaced. Clean all surfaces of kitchen, counters, walls, refrigerator tops, sides, fronts, etc.

Viola Stampede: Hwy. 223 and 62, Viola. Date of inspection March 26. Corrective action needed: Waitress handsink should be used for hands only. Provide paper towels at handsink for waitresses and customers. Wiping cloths should be stored in sanitizer when not in use. Sausage should be held at 135 degrees F or below 41 degrees F. Store raw meat below ready-to-eat foods in coolers. Need test strips for santizier. Dumpsters are required to have lids. A utility/mop sink is required to properly dispose of mop water. Raw meat in refrigerator is not covered. To comply with under cooked egg variance, menu should state consumption of raw or undercooked foods may result in an increased risk of food borne illness, especially for people with certain medical conditions.

Frederick's Restaurant: 2178 Hwy, 62/412, Hardy. Date of inspection March 27. Label dry good containers and lids. Permission to open is granted once above item is corrected.
