Arkansas Health Department Inspections

The following are the results of inspections conducted through April 22 by the Arkansas Health Department. The Ash Flat office conducts inspections in Sharp, Fulton and Izard counties.

Chuckwagon Restaurant: Hwy. 62/412, Hardy. Date of inspection March 27, 1:30 p.m. Corrective action needed: Replace light shields in walk in freezer. Store food off floor of walk in freezer. Ice maker should drain to sewer. Dispose of boxes in dumpster. Chop block is to be replaced. Above items are allowed 90 working days to come into compliance.

Lakeview Dinette: 200 Iroquois, Cherokee Village. Date of inspection March 27, 1 p.m. Corrective action needed: Wiping cloths improperly stored. Prep cooler is still not operating at 41 degrees Fahrenheit or below.

Subway of Ash Flat: 105 A Hwy. 62, Ash Flat. Date of inspection April 6, 1 p.m. Corrective action needed: Foods in hot hold should be maintained at 135 degrees Fahrenheit or above. When employees wash hands, soap and 20 seconds of scrubbing is required.

Pizza Hut: 142 Hwy. 62, Ash Flat. Date of inspection April 6, 1:35 p.m. Corrective action needed: Hand sink should be used for hands only. Store utensils handle up after washing. Ash tray with ashes at desk - violation of AR Clean Air Act.

Murphy USA: 105 Hwy. 62/412, Ash Flat. Date of inspection April 6, 1:30 p.m. Corrective action needed:Clean top of cappuchino machine and inside cabinets below. Clean inside ice chutes on soda fountain. Keep three compartment sink clean - even though not currently in use.

Price Chopper: 202 W. Main, Hardy. Date of inspection April 7, 10:15 a.m. Violations observed in previous inspection have been corrected at time of inspection.

Price Chopper Deli: 202 W. Main, Hardy. Date of inspection April 7, 9:45 a.m. Violations observed in previous inspection have been corrected or are currently addressed at time of inspection.

Negron's Bakery: 2110 Hwy. 62, Hardy. Date of inspection April 7, 11:15 a.m. Corrective action needed: Wiping cloths are required to be stored in sanitizer -- Repeat. Other violations observed in previous inspection are either corrected or are being addresses at time of inspection.

Highland Headstart: 1698 Rebel Drive, Hardy. Date of inspection April 7, 11:30 a.m. No violations observed at time of inspection. Meals are prepared at Highland Elementary.

China Garden: 2002 Hwy. 62, Hardy. Date of inspection April 7, 1 p.m. Items observed in previous inspection have been corrected.

Subway of Ash Flat: 105A Hwy. 62, Ash Flat. Date of inspection April 7, 1:15 p.m. Violations observed in previous inspection have been corrected at time of inspection.

Melbourne High School: 2270 LaCrosse, Melbourne. Date of inspection April 8, 12 p.m. Violations observed in previous inspection have been corrected at time of inspection.

Oxford One Stop: Hwy. 9 and Hwy. 354, Oxford. Date of inspection April 8, 1:40 p.m. Violations observed in previous inspection have been corrected at time of inspection.

Salem Dairy Queen: Hwy. 62 E., Salem. Date of inspection April 9, noon. Violations observed in previous inspection have been corrected at time of inspection.

South Fork Café: Hwy. 62 W., Salem. Date of inspection April 9, 1 p.m. Corrective action needed: Store wiping cloths in sanitizer when not in use. Do not store burger press on towels. Hands should be washed (or tongs used) before handling ready to eat foods after placing raw hamburgers on the grill. Do not store utensils in water between uses. Store ice scoop on sanitized surface. Clean well behind grill area. Store raw meat (steaks) away from and below ready to eat foods (tomato and lettuce).

Dollar General: Hwy. 62 W., Salem. Date of inspection April 9. Corrective action needed: Canned goods without proper labels should not be sold. Need hand wash sign in men's restroom.

Cornerstone Café: 1 Nursing Home Drive, Sidney. Date of inspection April 13, 12:30 p.m. Corrective action needed: Home canned jelly and BBQ sauce is not approved for use in food service. Paint raw wood in dry storage. Jalapenos require refrigeration after opening. Vanilla from Mexico is not approved for food service. Beans are required to be stored at 41 degrees Fahrenheit or below. Insides of microwaves need cleaning. Replace missing tile in microwave room. Sausage is required to be stored above 135 degrees Fahrenheit in hot hold. Utensils should be air dried out of direct contact with towels. Self closing doors and a covered waste can are required in women's restroom. Hot water is required at hand sinks. A hand wash sink is required in the food prep area -- contact a plumber to install. Need light shields above kitchen counter. Any time the location of the kitchen changes or is extensively remodeled; a plan review should be submitted to ADH. A variance is required to serve under-cooked eggs -- will do at follow-up inspection. Will be required to post in menu "consumption of raw or under-cooked eggs may increase the risk of food-borne illness, especially in persons with certain medical conditions."

Peace Lutheran Preschool: 12 Choctaw, Cherokee Village. Date of inspection April 14, 1:30 p.m. Corrective action needed: Microwave needs cleaning.

Lakeview Dinette: 200 Iroquois, Cherokee Village. Date of inspection April 14, 11:10 a.m. Violations observed at time of inspection have been corrected at time of inspection.

Cherokee Village Market: 201 Cherokee Village Mall, Cherokee Village. Date of inspection April 14, 10:40 a.m. Corrective action needed: Microwave needs cleaning. Clean vent hood and ceiling above pizza oven. Sausage is required to be stored above 135 degrees Fahrenheit or below 41 degrees Fahrenheit. Three compartment sink must be used to wash, rinse and sanitize. Dairy creamer should be stored at 41 degrees Fahrenheit or below. Label all dry goods. Need variance for eggs -- will do on follow-up inspection.

Spring River Village Fountain Place: #2 Chickasaw, Cherokee Village. Date of inspection April 14, 10:30 a.m. Corrective action needed: Hand sink should remain accessible at all times and used only for hands - corrected on site. Rings should not be worn during food prep. Label spray bottles with contents. Wiping cloths should be stored in sanitizer equal to 50 parts per million of bleach.

Fredericks Restaurant: 2178 Hwy. 62, Hardy. Date of inspection April 14, 1 p.m. Corrective action needed: Maintain ice to product level in ice baths to maintain foods at 41 degrees Fahrenheit or below. Store wiping cloths in sanitizer when not in use. Raw bacon should be stored at 41 degrees Fahrenheit or below 72 degrees Fahrenheit at time of inspection -- corrected on site. Do not cover foods with towels. Raw pork chops, steak stored above ready to eat foods -- corrected on site. Foods (gravy) should be chilled to 70 degrees Fahrenheit within two hours and 41 degrees Fahrenheit or below within another two hours. Use ice baths, smaller containers in freezer, etc. Provide scoops with handles for dry goods dispensing. Do not store knives in wooden blocks -- cannot properly clean within the slits -- corrected on site. Do not dry plates and glasses on towels. Store chemicals separate from foods and not on top of ice machine -- corrected on site. Repair dishwasher to provide sanitizer equal to 50 parts per million of bleach -- corrected on site. Personal items (hats and sweatshirts) should be stored separately from food -- corrected on site.

Chuck Wagon: Hwy. 62/412, Cherokee Village. Date of inspection April 14, 1:30 p.m. Follow-up to Complaint: Complaint filed stating they have observed children in food prep area. This has been addressed in a corrective action plan. Children were not observed at time of inspection. However, a walker and a child carrier were located in the food prep area. Failure to comply with the corrective action plan will result in a hearing before the State Board of Health.

ICCS Baseball Concessions: Hwy. 9, Brockwell. Date of inspection April 15. No violations observed at time of inspection.

Wood's Riverbend: 80 Main, Mammoth Spring. Date of inspection April 16, 10:30 a.m. Violations observed in previous inspection have been corrected at time of inspection.

Sonic of Salem: Hwy. 62 and Hwy. 9, Salem. Date of inspection April 16, 9:45 a.m. Corrective action needed: Food is still being stored in unused restroom. Prepared foods should be separate from washing area. Store dry goods away from wash sink. Repair light shield in walk in cooler. Dumpster still needs to be replaced with one that has lids. All critical violations have been corrected at time of inspection.

The Dawg House: 438 Hwy. 63, Mammoth Spring. Date of inspection April 16, 10:45 a.m. Violations observed in pervious inspection have been corrected at time of inspection.

Hog Wild Pizza: 201 Main, Mammoth Spring. Date of inspection April 16. Utility sink has been installed at time of inspection. No new violations observed at time of inspection.

Spring River Superette: Hwy. 63, Mammoth Spring. Date of inspection April 16, 12:30 p.m. Corrective action needed: Hand wash sink should be used for hand washing only.

Mammoth Country Music Hall: Music Hall Drive, Mammoth Spring. Date of inspection April 16, 1 p.m. Corrective action needed: Provide paper towels at hand wash sink in kitchen.

Mammoth Spring Senior Center: 9th and Archer, Mammoth Spring. Date of inspection April 16, 11 a.m. No violations observed at time of inspection.

Gamers Connection: 811 Bend Drive, Horseshoe Bend. Date of inspection April 17, 12:30 p.m. Corrective action needed: Potatoes once baked are potentially hazardous and should be stored at 41 degrees Fahrenheit or below or above 135 degrees Fahrenheit. Paint raw wood shelves below counter. Store wiping cloths in sanitizer equal to 50 parts per million of bleach when not in use. Store ice scoop handle up in ice or on a sanitized surface. Employee drinks need lids and straws -- corrected on site. Coffee was being used as an ingredient.

The Lunch Box: P.O. Box 276, Oxford. Date of inspection April 17, 1 p.m. No violations observed at time of inspection.

Flash Market 105: 2511 Hwy. 63, Hardy. Date of inspection April 20, 2 p.m. Corrective action needed: Store utensils handle up after washing. Store wiping cloths in sanitizer when not in use. Medicine should not be stored above food prep area. Clean top of freezer by fryers. Replace missing light shields in walk in cooler. Clean inside of microwave. Provide soap at hand wash sink behind counter.

Beach Club BBQ: 5 Jojo Lane, Hardy. Date of inspection April 20, 1:30 p.m. Corrective action needed: Need scoops with handles for dry goods (corn meal). Do not store knives in cracks between equipment -- corrected on site. Store Ibuprofen separate from food prep area -- corrected on site.

Giefers Possum Trot: 1416 Hwy. 69, Melbourne. Date of inspection April 21. All violations observed in previous inspection have been corrected at time of inspection.

Carriage Room: 900 4th Street, Horseshoe Bend. Date of inspection April 21, 2:30 p.m. Corrective action needed: Consumer advisory needs to be posted either in menu or on tables -- correct by next inspection. Critical items have been corrected at time of inspection. Correct non-critical items from previous inspection by next inspection. Facility was not operating at time of inspection.

Main Street Pizza: 108 E. Main, Hardy. Date of inspection April 22, 1 p.m. Corrective action needed: Lower temperature of refrigerator to 41 degrees Fahrenheit or below. Store raw chicken below ready to eat foods. Repair leak beneath hand sink in kitchen. Store wiping cloths in sanitizer when not in use. Lower temp of prep cooler to 41 degrees Fahrenheit or below -- coolers should not be turned on and off -- cooler was off at time of inspection. Clean inside of microwave. Crack beneath door should be sealed to block the entry of pests and other contaminants. Clean the mixer. Can opener blade should be cleaned and sanitized daily. No hot water at hand sink in dish room -- valve turned off. Do not turn valve off -- replace valve. Pizza spatula needs cleaning and should be stored on sanitized surface. Need sanitizer test strips. Need trash cans with lids outside.

Hattie's Ice Cream Parlor: 640 Main, Hardy. Date of inspection April 22, 1:30 p.m. Corrective action needed: Store wiping cloths in sanitizer when not in use. Dipper well should be operating correctly at all times of operation. Provide paper towels at hand sink in employee restroom -- corrected on site.

Highland Wee Care Enrichment: 1693 Hwy. 62/412, Hardy. Date of inspection April 22, 2 p.m. No violations observed at time of inspection.

Highland Wee Care Daycare: Hwy. 62/412, Ash Flat. Date of inspection April 22, 2:15 p.m. Corrective action needed: Thawing should be done fully submerged under constant flow of water, or in refrigerator.
